Report information or refer individuals for assistance by completing the reporting/referral form below that best matches your concern. Reports and referrals go directly to the appropriate office. Please include your name and contact information; you may be contacted as part of the follow-up. Anonymous reports are accepted, yet they often lack necessary information for effective resolution. Academic misconduct reports contain allegations of violations of the Code of the University's Academic Honesty Policy. Reports may include, but are not limited to, plagiarism, cheating, collusion, and improper use of technology.
Report Academic MisconductDisruptive behavior or a policy violation includes allegations of violations of the Statement of Student Responsibilities. The reports may include, but are not limited to, behaviors such as disorderly conduct, hazing, harassment, physical violence, use of alcohol or drugs, or other violations of university policies, regulations, or rules.
Report Disruptive Behavior or Policy ViolationConcerning behavior may include mental health issues; inappropriate display of anger or emotion; suicide ideation or attempt; significant changes in appearance, hygiene, or behavior; housing/food insecurity; written work that reflects these issues, and other concerns that may impede a student's success. If the student poses an immediate threat to self or others, please call 911 or University Security at 507-389-2111.
Report Care and Concern for an IndividualConduct may include an actual, attempted, or threatened sexual act without the other's affirmative consent, dating and relationship violence, stalking, intentional and unwelcome touching, indecent exposure, voyeurism, aiding acts of sexual violence and other concerns.
Report Sexual or Interpersonal MisconductConduct may include discrimination (differential treatment), discriminatory harassment (unwelcome conduct/communication that has a negative effect), and/or sexual harassment (unwelcome conduct/communication of a sexual nature that has a negative effect) that is based on an individual’s protected class status.
Report Discrimination or Discriminatory HarassmentSubmit a report to the specific department responsible for the area of your concern OR submit a formal grievance to the Vice President for Student Affairs' office.
Submit a Student Complaint or GrievanceReport any concerns not covered by other reporting options, and related to faculty or staff.
Report a Faculty or Staff ConcernThese incidents include language, images/symbols, conduct, or actions that are hostile, intimidating, or prejudiced in nature towards an individual or group based on their actual or perceived membership of a protected class. Protected classes include race, religion, disability, national origin, sex, marital status, familial status, age, sexual orientation, and gender identity.
Report a Bias IncidentAttendance issues; low to no engagement in course; missing work; low quiz/test scores; low course grade; and general course progress concerns.
Report a Student Academic Success ConcernMake an anonymous report to University Security.
