Temporary Health Condition

If more than two weeks of class is missed due to illness, Accessibility Resources offers support to students with temporary documented health conditions. Services are extended only for the duration of the limitations associated with the illness. Students experiencing temporary illnesses (less than two weeks) are responsible for contacting their course instructors.

Students should provide documentation of their temporary illness including diagnosis and a summary of the functional limitations. After reviewing your documentation and meeting with a staff member, the appropriate supports are determined based on how you are functionally limited in the academic setting.

This temporary illness form may be completed and returned by your medical provider for documentation purposes.


In compliance with Title IX of the Education Amendments Act of 1972, Minnesota State University, Mankato prohibits discrimination in its educational programs and activities on the basis of sex. This includes pregnancy and related conditions. Students who are pregnant may request adjustments based on general pregnancy needs or ADA disability accommodations based on a pregnancy-related complication.
