The following forms can be filled in then submitted to the appropriate office indicated on the form.

Undergraduate

Academic Forgiveness

Academic Forgiveness gives an undergraduate student a one-time opportunity to establish a new Grade Point Average (GPA).

Common Market

The Common Market program has different forms depending on your situation. The link will take you to the information page with the available forms.

Credit by Exam

This form is used when a student is attempting to earn university credit by completing an exam. This process is facilitated by departmental faculty. You must discuss this opportunity with the faculty member before completing the form and payment must be made before the exam is completed.

Enrollment Verification

This form is used when enrollment verification is required and the asking agency has refused the verification through the National Student Clearing House.

Diploma Replacement

Use this link to be directed to our online ordering form to request a replacement diploma. 

Grading Method Change Appeal

This form is used to change grading method (grade, audit, or P/N) after the specified date.

Immunization

This form is used to provide the University with your immunization information.

Incomplete Contract

This form is used when a student requests an Incomplete grade from an instructor. It lays out what work needs to be completed by the agreed upon date for the student to earn a grade. Failure to comply with the contract results in a grade of F or NC after one semester. For Spring courses, the timeframe to complete the work is the end of Fall semester.

Information Release

This form is used to grant authorization for a third party to have access to financial and/or enrollment information.

International Student Non-Immigrant/Immigrant Intent

This form is used by non-citizens as proof of filing for immigrant/permanent resident status in order to be considered for in-state tuition.

Medical Verification for Registration Appeal

This form is used to appeal registration holds when a mental health or physical condition prevented the student from completing coursework.

MN Prosperity Act Appeal

 

Official Transcript Request

All official transcripts are ordered through the National Student Clearinghouse website. 

Personal Data Change

This form is used to change your name when there is a legal change in your name.

Preferred Name Change

Students are able to indicate their preferred name through eservices. Once logged into eservices from the student dashboard select Account Management and then select Name and Address Info. 

Not to be used to change your legal name.

 

Registration Appeal

This form is used to appeal late adds, drops, withdrawals, etc.

Repeated Course

This form is used to notify Registration and Academic Records of a repeat transfer course. 

Repeated Course Policy Appeal

This form is used to appeal the two repeat policy. An approved appeal lets the student take a course a fourth time.

Residency Reclassification Request

This form is used by students who wish to be considered for reclassification as Minnesota residents for tuition purposes.

Senior Citizen Status Request

This form is used by students that have never attended Minnesota State Mankato and are over 62 years old.

Student Athlete Add/Drop Request

This form is used to allow student athletes to add or drop courses, as they have a hold preventing them from going below 12 credits per semester.

Student Data Privacy Request

This form shows what information the University can release, as well as provides an option to limit release of information.

Transfer Appeal/General Education, Diverse Cultures, & Writing Intensive Substitution

This form is used to appeal transfer credit evaluation. It is also used to request a Minnesota State Mankato course be included in the General Education, Diverse Cultures, or Writing Intensive Requirements.

Undergraduate Application for Graduation

This form is used to apply for graduation. It should be filled out after you register for your final semester.

Undergraduate Credit Overload

This form is used to request permission to enroll in more than 18 credits in a semester.

Graduate

This form is used to request permission to enroll in more than 12 credits in a semester.

Auxiliary Course Proposal

This form is used to create a course that can be taught without going through the curriculum committee.

Open Option Grading Request

This form is used to request "open-option" grading for 300/400 level courses.

Variable Sub-Title Course Request eform

This form is used to request permission to offer a workshop, in-service course, or similar group experience.
