The following forms can be filled in then submitted to the appropriate office indicated on the form.

Academic Forgiveness- Undergraduate

This form is used to for undergraduate students to apply for Academic Forgiveness, which provides a one-time opportunity to reset their Grade Point Average (GPA) under specific conditions.

Application for Graduation

This form is used to apply for graduation. It should be filled out after you register for your final semester.

Common Market-GAC

The Common Market Program allows undergraduate students already admitted to Minnesota State University, Mankato to take courses at Gustavus Adolphus College (GAC) without needing to apply for admission to GAC.

Common Market- Bemidji or Metro

The Common Market Passport Program allows undergraduate students already admitted to Minnesota State University, Mankato to take courses at other Minnstate Universities. Only Bemidji State University and Metro State University require a Common Market request.

Credit for Prior Learning

This form is used when a student seeks to earn university credit using Credit for Prior Learning.

Credit Overload-Undergraduate

This form is used to request permission for undergraduates to enroll in more than 18 credits in a semester.

Credit Overload-Graduate

This form is used to request permission for graduate students to enroll in more than 12 credits in a semester.

Course Repeat-Undergraduate

This form allows an undergraduate student to request that a higher grade earned in a repeated course replace a lower grade previously received. It is specifically intended for repeated courses that are transfer courses or topics courses.

Enrollment Verification

This form is used when enrollment verification is required and the asking agency is unable to use the National Student Clearinghouse.

Diploma Replacement

Connects students to our diploma vendor, Paradigm, to order a diploma replacement.

Grading Method Change Appeal

Used when a student is appealing to change their grading method in a course after the deadline (i.e. changing from Pass/No Credit to Letter Grade).

Immunization

Directs students to the Student Health Center to understand the immunization requirements and forms needed.

Incomplete Contract

This form is used when a student requests an Incomplete grade from an instructor. 

Information Release

Directs students to the Student Authorization or Release of Information process at MSU. 

Medical Verification for Registration Appeal

This form should be completed and attached to any registration appeal that involves a mental or physical health condition.

Notice of Immigration Status Change

This form is used by non-citizens as proof of filing for immigrant/permanent resident status in order to be considered for in-state tuition.

Official Transcript Request

This link is used to order all official transcripts through the National Student Clearinghouse. 

Personal Data Change Form

This form is used to update your legal name, SSN, date of birth, and personal email address.

Preferred Name Change

Links to E-services where students can update their preferred name. Updating your preferred name does not change your legal name, which will still be used in official university documents.

Registration Appeal

Students may submit a registration appeal when they have missed an academic deadline due to circumstances beyond their control (such as a medical issue or a university error) to add, drop or withdraw from a course. This form is also used for appealing to change a grading method (i.e. Pass/No Credit to Letter Grade).

Repeated Course Appeal

This form is used when a student is appealing to attempt a course for the 4th time.

Resident Reclassification Request

This form is used by students who wish to be considered for reclassification as a Minnesota resident for tuition purposes.

Senior Citizen Status Request

This application is used by students over the age of 62 who are looking to be classified as senior citizens to complete coursework at a reduced rate.

Student Athlete Registration Change Request

Student athletes use this form to add, drop, or withdraw from courses after the add/drop deadline. 

Student Data Privacy Request

This form allows students to update or change their Data Privacy Status at MSU.

Transfer Credit Appeal

This process is used to appeal a transfer credit evaluation, including transfer work within General Education, Diverse Cultures and Writing Intensive.
