Health, Saftey and Required Forms
Minnesota State Mankato requires specific forms for minors participating in on‑campus camps, all of which are securely collected through our online camp management system.
Camps hosted through Athletics and those listed on campmsu.com automatically collect these forms during registration.

Every camp operating on campus follows established safety and operational procedures, including:

- Medication management and secure storage
Camp Medication Distribution Procedure
Distribution of participants’ medicines by program staff should be handled under the following conditions:
Read More Read Less- Program staff shall be responsible for reviewing all forms and assessing needs of each program participant.
- The participant’s family provides the medicine in its original pharmacy container labeled with the participant’s name, medicine name, dosage and timing of consumption. Over-the-counter medications must be provided in their manufacturers’ container.
- Program staff shall keep the medicine in a secure location, and at the appropriate time for distribution shall meet with the participant. - Checklist
- The program staff member shall allow the participant to self-administer the appropriate dose as shown on the container.
- Parent(s) and/or guardian(s) are expected to make arrangements for the administration of any medicine that the participant cannot self-administer.
- Devices for the self-administration of medications which are prescribed by a physician may be carried by the participant during program activities (e.g: “epi” pens and asthma inhalers).
- Over-the-counter medications can only be administered with prior approval. Participants can self-administer over-the-counter medication that they bring themselves.
camper Check-In and Check-Out
All camps are required to use a check‑in process to ensure accurate camper information. Most Minnesota State Mankato camps use the Ryzer camp management system—please bring your Ryzer confirmation email, as staff will scan the QR code to check campers in and out.
If your camp is not using Ryzer, the camp manager should communicate the check-in and check-out locations.
